Driver Authorization

All club sport participants, coaches, and volunteers are required to be authorized by the Office of Risk Management to drive other individuals outside of themselves. Individuals driving only themselves DO NOT need to be authorized as drivers. Refer to the Driver Authorization process below for step-by-step instructions for becoming an authorized driver. All documentation must be submitted to the Club Sports Administration, who will then work directly with the Office of Risk Management for the submission. Please note that students in multiple club sports only need to complete this process ONCE.

Driver Authorization Process

1. Acknowledging Driver Agreement

  • Read, acknowledge, and complete the Driver Info tab on your DSE registration
  • Input your driver license expiration date
  • Agree to driving responsibilities and guidelines

 

2. Complete the Driver Authorization Form

  • Attach legible photocopy of driver's license
  • Complete Drivers Information section
  • Please indicate whether you are a: Student, Employee (paid contingent coaches), or Volunteer (volunteer coaches)
  • Leave Department Section BLANK
  • Email completed forms to cwadams@stanford.edu with "Secure:" in the subject line.

 

If you are (a) an out-of-state driver or (b) have held a California driver's license for less than THREE years:

  1. Check the appropriate box in the Driver Authorization Form
  2. Submit your out-of-state/in-state driving record with your form. Please note that obtaining your driving record may come with a nominal fee, depending on the state of issuance. It is left to your club's discretion if they will reimburse you directly for obtaining your driving record.
